Fulfillment OST request for support around damage form

The Fulfillment Operations Subteam is recommending a new process for reporting damaged AFN items to lending institutions. The process would preserve damaged item circulation history via Microsoft Form & Power Automate. The form would help improve communication and retain data between UC campuses regarding damaged item. Information collected would include patron circulation records which is anonymized in Alma when an item is checked in.

 We would like to ask the Operations Team for support and guidance for a solution to find a host for the Microsoft form.
